Please complete and return this re-enrollment form via mail or in person to your child’s teacher or the
main office in order to renew your child’s enrollment for the 2017-2018 school year at HSAY.
Deadline for re-enrollment is Friday, February 24, 2017.
The consumable material fee of $25.00 will be waived if you turn this form in by Friday, February 24!
Students who turn this form in by the deadline will receive a bag of school supplies on the first day of
the 2017-2018 school year.
We will also be having a Spirit Week for all students who turn this form in by the deadline.
If you do not turn this form in by Friday, February 24th, there is a non-refundable consumable material
fee of $25.00 per child. For the convenience of families with multiple children the $25.00 will apply to
the first child and additional siblings will be $15.00 per child. Payment may be made in cash, money
order, or credit card.
